The Rural Health Transformation Program is a $50 billion federal program created by Public Law 119-21 and administered by CMS through the states. In Illinois it is managed by the Department of Healthcare and Family Services (HFS). Years 2 through 5 are not guaranteed: CMS renews each state on satisfactory progress, so the reporting your hospital does now is part of Illinois's case for continued funding.

Three grant opportunities open to eligible hospitals

The Illinois Critical Access Hospital Network (ICAHN) is the largest sub-awardee of Illinois's first-year RHTP funding. It administers three programs for eligible critical access and rural hospitals. Applications run through the state's Euna grants portal.

Technology Transformation

Cybersecurity and physical security, telehealth expansion, EHR optimization, automation and artificial intelligence (AI), data analytics, consumer-facing digital tools, shared infrastructure, and clinical technologies.

First step: a digital readiness and needs assessment for all 78 eligible hospitals, coordinated by ICAHN.

$14 MILLION

Disease Prevention & Chronic Disease

New or expanded evidence-based programs: community health worker outreach, patient education, remote patient monitoring, and structured chronic disease management.

Required: local health priorities drawn from your Community Health Needs Assessment, county I-PLAN, and local data.

Rural Workforce Investment

Recruiting, educating, and retaining clinicians: tuition assistance, clinical training, preceptors, retention incentives, recruitment campaigns, and apprenticeships.

After your digital readiness assessment

The assessment tells you where you stand. It does not tell you what to buy, or how.

ICAHN's statewide assessment gives every eligible hospital a current-state baseline: maturity scores, gaps, risks, and dependencies. By design, it stops there. The assessment scope excludes vendor recommendations, technology roadmaps, and implementation, and leaves those decisions to each hospital and its designated advisors.

That next phase has a hard clock on it. ICAHN has said Year 1 technology funds must be spent by 30 September 2027. Hospitals that move from assessment to a defensible plan quickly will be the ones ready when applications open.

Ten grants, three routes to your hospital

Illinois funded eight RHTP initiatives as ten grants. Only one was a competitive application directly by hospitals. The others reach a hospital through a sub-award or through a partner organization, which means their value depends on how each recipient designs its program.
